Thank you for sharing this worksheet, I like the way you have made it look (in fact, I have copied your idea of using a notebook as a background, I hope you don't mind).
I would like to point out a few things that made my students give a "wrong" answer in the last exercise, in case you can spare the time to change that:
In a) you could start "She asked me..." so that the student knows which subject to use. I noticed you were using "me / I" when the original sentence was addressed to "you", but that's not clear in the instructions. Otherwise you would have to admit as correct all the possibilities, like "we" (you did this in f), "they", "he", "she",... but I find this leaves students a bit frustrated because they don't really know which is the "correct" one.
